ELECTRONIC FUNDS TRANSFER AUTHORIZATION EFT AGREEMTN FORM

I hereby authorize Knies Enterprises Inc. DBA Energy Tanning to make monthly withdrawals from credit card account listed below in the amount specified below until I submit a written cancellation sent by certified mail with return receipt requested to the corporate office.  If I fail to fulfill my obligation of the six (6) month minimum agreement or try to cancel my membership prior to the end of the obligation, I also give my authorization to Knies Enterprises Inc. DBA Energy Tanning to withdrawal the remaining balance from any account necessary to fulfill amount owed for the minimum time of the contract which is six months and any costs incurred to recover the amount to fulfill the six month minimum obligation.  I also understand that this agreement does not end in six months, it is just a six month minimum and I must cancel the membership in writing via certified mail. I also understand that it could take up to thirty (30) days for my cancellation to be taken out of the system and I will have tanning privileges through the end of the that time period.

INHOUSE BALANCES/NSF FEES: In addition, I grant Knies Enterprises Inc. DBA Energy Tanning or its agents the right to recover any incorrect Electronic Funds Transfer amount which results in an erroneous underpayment of amount owed which could be caused by an entity involved in the monthly debiting process by debiting my account to the extent of such underpayment. 

In addition, I understand that if my credit card is declined for any reason, I may be subject to a $25.00 NSF fee.

Also, I grant Knies Enterprises Inc. DBA Energy Tanning the right to make additional charges with my consent for any products or services which I agreed to purchase and place on an "In-house" status if I have not paid for those products or services within 30 days of receiving the product or service.

ADDITIONAL TERMS: The terms of this agreement supercede all others I may have with Energy Tanning but does not relieve my obligations as to minimum time nor dollar amounts from the original agreement. 

Failure to sign this agreement may result in forfeiture of my tanning privileges buy does not relieve me of my obligation from any other contract I may have on file with Energy Tanning. 

CHANGES/UPDATES: I understand that this form may be used to merely update the current records of Energy Tanning and if I am an existing EFT client, this will not restart my six (6) month minimum nor add additional time to that minimum. 

All time accrued to this point will be credited toward my six month minimum.

CANCELLATIONS:  This authorization is to remain in force until the company has received written authorization via certified US Mail from me 30 days prior to its termination or change. Cancellations MUST be made in writing and sent via certified mail to:
